Box 1778, Silverdale, Washington 98383.  BLOCKS’ main telephone number is (844) 717.0642.  If you would like a copy of BLOCKS'  financial, licensing or charitable purposes(s) information please send a request indicating the information you desire, along with a self-addressed stamped envelope or an email address to which you would like the information sent, to BLOCKS, P. O. Box 1778, Silverdale, Washington 98383, Attn: Disclosure Requirements.  BLOCKS is exempt from federal income tax under 26 U.S.C. Section 501(c)(3), and is a non-profit organization eligible to receive tax-deductible donations.  Each donation to BLOCKS is tax-deductible to the extent allowed under applicable law, and represents a contribution to its entire mission, which is to work with childcares, offices of emergency management, and the community to keep kids safe during disaster.  Registration in any State/DISTRICT does not imply an endorsement of BLOCKS by that State/DISTRICT, OR ANY OFFICER OR EMPLOYEE OF THE State/DISTRICT.
